Information Sheet for OWIUS 2013 Study

Overall Aim of the Project
The main aim of this project is to conduct a follow-up study on student well-being that we completed in 2012 and identify how student well-being has changed one year later and factors that might be influencing well-being and mental health in students who were at Residential Colleges and University Flats in 2012. We are also interested in the well-being of new students at Residential Colleges and University Flats in 2013. The results from this study may lead to implementing a set of helpful tools for Residential Colleges and University Flats, including online training and/or therapies in the future.
Participation Criteria
All students who took part in the OWIUS 2012 well-being study and this year’s students from Otago Residential Colleges and the University Flats are being invited to participate in this online (follow-up) well-being study.
The personal information that we require from you are your
· Name
· Email Address
· Age
· Gender
· Residential College (a unique code will be assigned for each College)
· Role (Student, RA, Kiwi Host)
· Ethnicity
Data Collection
We will invite you to complete one short online survey that includes questions on your own well-being and your sense of belonging to various groups. Each participant will have a unique identification number that will be used instead of their name on the online forms. Your contact details (e.g. name and email address) will be stored in a separate and secure database from your responses to the online well-being surveys. Your responses will be kept strictly confidential. Only members of the research team will be able to access the responses you give to this study. All data will be securely stored in encrypted databases on the research team’s computers at the Department of Preventive and Social Medicine. All paper copies related to this project will be securely stored in locked filing cabinets at the Department of Preventive and Social Medicine.
What is Being Asked of You
Initially, you will be invited to register for the study by reading the Information Sheet and filling out a Consent Form. On submitting this form, you will receive an automated email that will include a unique identification number and link for the online well-being survey. The identification number, which is unique to each participant, will be used in place of your name for the online survey. The first part of the questionnaire asks for basic demographic details about yourself, such as age and ethnicity. The second part is an evaluation of your own well-being and identification with social structures. This should take about 15 minutes altogether.
As a token of appreciation for taking part in this study, two iPad minis, one each for the 2012 and 2013 OWIUS study groups will be raffled. The study identification numbers of all student participants who complete the online well-being survey will be entered in the draw to win these two prizes.
Withdrawal
Participation in this project is entirely voluntary. You are also free to withdraw from the study during the course of the study without consequences.
Questions
There are not expected to be any known risks to yourself associated with completing the two online well-being surveys. However, if at any time should you become concerned about the well-being of yourself or someone you know, we suggest that you discuss your concerns with the Warden or Deputy Warden at your Residential College and/or, contact Student Health Services on Campus (Freephone 0800 479 821; Tel 64 3 479 8212; Email student-health@otago.ac.nz) or your Doctor/primary health care provider.
